New Patient Information

Please call our office at (209) 736-2030 to make an appointment. Office visits may need to be rescheduled if a doctor is involved in a medical emergency. You will be notified well in advance by our office, whenever possible, if appointment changes are necessary. If you need to reschedule your appointment time, as a courtesy notify the office as soon as possible, at least 24 hours in advance, to avoid 'failed appointment' charges and to make the appointment time available for others.

Information to Bring To an Office Visit: 

1 Please bring your insurance cards, list of medications you are currently taking with dosage, any medical records, or test results to your visit with us.

2. If your insurance requires a referral, please come prepared with your Primary Care Physician Referral and be prepared to pay your copayment as required by your insurance company.

3. If you are a new patient to our office, please arrive 20 minutes early to complete the paperwork. If an existing patient, please arrive 10 minutes early.

How do I reach my physician after hours?
We request that you make routine calls, appointments, and requests for prescription renewals during our regularly scheduled office hours. For emergency calls, please call the Dr. Lovett's pager: 209-428-0266
How do I get my prescription refilled? 
All prescriptions and authorizations for refills should be requested during our  normal office hours. Please be certain you have necessary refills at the time of your office visit, as it is our policy to not refill medications in the evenings or on weekends. Your regular medications for non-orthopedic problems should be refilled by your family physician. 
How do I get copies of my medical records?
We need a signed consent form from you to release your medical records. The release must include your name, the date, and your doctor's name and the address where they are to receive the records.

What can I expect during the first consultation?
Our receptionist will ask you to fill out a patient history form giving us important information about your general health, and other necessary information for our Doctors to effectively treat your orthopedic problems. We encourage you to bring copies of your insurance cards, insurance forms, or any other outside medical records, or x-rays that can help us evaluate your condition.

To better acquaint the doctors with your health problems, we encourage you to write down questions you would like to ask the doctor. You will find that the doctors and their staff are most willing to spend the time necessary to make your visit informative.

How to Prepare For Your Examination or Visit?
At the time of your initial appointment, please bring all medical records pertinent to your orthopedic problem, a list of medicines and dosages, a list of your medical illnesses, and previous surgeries and their dates.

Fees
Our fee for a first time visit or a new problem evaluation varies according to the complexity of your problem, and is higher than for a follow-up visit. Please understand that our charges are "usual and customary fees" for medical services provided by Orthopedic Surgeons in California, despite what your insurance company may say in their explanation of benefits to you.

Payment for your office visit is expected at the time of service. If we are participating providers for your insurance carrier, we will bill them for you. You will be responsible for any co-payments at the time of your office visit. If you anticipate a large bill, a payment program may be worked out with the office manager.
